Our holistic approach to care includes a licensed Adult Day Care and a variety of mental wellness services to meet each individual’s needs.

Aging True Home Health
Aging True Home Health is a Medicare certified home-health agency within Aging True Community Senior Services and provides intermittent skilled care to adults age 21 or older when ordered and under the care of a physician. Our skilled clinicians work with the doctor and patient to create a plan of care and help the patient reach their health goals. The services provided or arranged by Aging True Home Health include the following:

The Mental Wellness services provided by Aging True are tailored to the needs of seniors and their caregivers, as well as individuals who are home-bound, disabled, or otherwise unable to access traditional methods of receiving mental healthcare.

Adult Day Health Care
Aging True operates the Adult Day Health Care located at the Clayton and Mildred Revels Senior Center in Green Cove Springs. Our center brings peace of mind to caregivers while offering a secure environment for socialization, health monitoring, nutritional guidance, and activities under the supervision of a Registered Nurse.
